Rescue Partner Agreement and Application Kitty Devore Rescue Network AgreementI have the authority to make decisions for the 501c3 nonprofit rescue group I represent or a rescue group with an EIN number through the Secretary of State of the state my rescue group is based and our 501c3 with the IRS is pending.* I DO have the authority I DO NOT have the authority I understand that by choosing to become a rescue partner with the Kitty Devore Rescue Network, I will be able to request the assistance of KD in my efforts to rescue cats from Devore Shelter in San Bernardino, California, but that in doing so I assume all responsibility for the care of the cat(s) once he or she is pulled from the shelter. This responsibility would include possible payment of shelter pull fees when a Rescue Only puller is not available to assist or Kitty Devore Rescue’s funding resources are depleted, arrangement and full financial funding of the cat’s vetting, additional medical expenses when necessary, a rabies shot, health certificate and possibly heart-worm test if the cat will be crossing the California state border into a state that requires these necessities before a cat can be allowed into the state, boarding, transportation arrangements and costs and any other aspect of rescue, rehabilitation and rehoming costs that arise in the process of the rescue effort.* I understand I do not understand Fundraising for cats pulled for rescue partners is to be done on the rescue partner’s own social media platforms. They will not be allowed in the Kitty Devore Rescue Network or on the Kitty Devore Rescue public Facebook page beyond posting a rescue effort and the rescue partner’s donation link on photo threads for that specific cat.* I understand I do not understand KD does not expect the rescue partner to vet the cats pulled for them in accordance with KD vetting policies. If certain vetting procedures need to happen prior to transport to the rescue partner and the rescue partner specifically requests our assistance to help them with this, it will be at a facility that the rescue partner approves of that is within our ability to transport the cat to and for a fee that the rescue partner feels reasonable and within their budget. KD will never insist on a rescue partner choosing a facility that we work with or pay above and beyond what their budget can afford or what they are willing to pay for certain procedures.* I understand I do not understand If a cat is not sent to be spayed or neutered at the off-site shelter veterinarian contracted by Devore Shelter, Devore Shelter requires the submission of a certificate of sterilization within 30 days of the date the cat is pulled from their facility. Failure to submit certificates or letters of extension or confirmation of previous sterilization procedures can result in the suspension of Kitty Devore pullers and is detrimental to our rescue efforts so we do ask that rescue partners be in strict observance and compliance of our certificate and letter submission requirements.* I understand I do not understand As a rescue partner, I understand that it is my responsibility to ensure the cat I have asked to be rescued on behalf of the rescue group I represent be spayed or neutered, that a certificate for the procedure be obtained or that a certificate or letter is obtained by me from a licensed veterinarian stating the cat is already spayed or neutered or is too old, is not able to breed and that a sterilization procedure is not necessary. For mother cats with kittens, the time allotment may be extended to 60 to 90 days depending on the age of the kittens. The due date will be noted on the shelter paperwork* I understand I do not understand I will submit all certificates and letters of extension or letters of prior sterilization for non-pregnant or non-nursing moms and kittens over the age of 8 weeks old within a 30 day period from the date the cat was pulled from Devore Shelter through the Kitty Devore Rescue website’s private and password protected member area using the Certificate Submissions form.* I understand I do not understand Devore Shelter requires the submission of a death certificate in the event a cat dies before they can be spayed or neutered within the 30 day period from the date of rescue. If the cat I have asked to be rescued on behalf of my rescue group dies or his humanely euthanized within the 30 day period after they are pulled from the shelter, it is my responsibility to obtain a death certificate or letter from a licensed veterinarian stating the cat has passed away. I will submit all certificates and letters within a 30 day period from the date the cat was pulled from Devore Shelter through the Kitty Devore Rescue website’s private and password protected member area using the Certificate Submissions form.* I understand I do not understand If my veterinarian states it is their professional opinion that a kitten is too young, physically immature or too small to be sterilized and that it is preferable to wait to perform the procedure at a later date beyond the 30 day allowed period, I will obtain a letter of request for sterilization extension from my veterinarian. I will submit this letter of sterilization extension request within a 30 day period from the date the cat was pulled from Devore Shelter through the Kitty Devore Rescue website’s private and password protected member area using the Certificate Submissions form.* I understand I do not understand I understand if a pregnant cat is pulled for me at my request and I am choosing to allow the mom to give birth to her kittens, I have 30 days from the date of rescue to submit a letter of extension for spay of the momma cat from a licensed veterinarian to KD administrator, Renée Mc Elwee. This letter will be submitted to the administrators of Devore and Renée will then, in turn, respond back to me to let me know the extension has been approved and granted. Letters of extensions for pregnant moms should be sent through the Kitty Devore Rescue website’s private and password protected member area using the Certificate Submissions form. Note that the kittens born of a pregnant momma cat do not need to be submitted to Devore if they were born AFTER she was pulled from the shelter. There is no obligation for the rescue group partner to submit them to an administrator of Kitty Devore as proof of sterilization.* I understand I do not understand I understand if a nursing mother cat with kittens under the age of 8 weeks old are pulled for me at my request, I have between 60 and 90 days from the date of rescue to submit sterilization certificates for all members of the family through the Kitty Devore Rescue website’s private and password protected member area using the Certificate Submissions form. The time frame for submission may vary depending on the age of the kittens at the time they were pulled from the shelter. The due date will be noted on the shelter paperwork.* I understand I do not understand I understand Shelter paperwork is not immediately provided to rescue partners as a general rule. Kitty Devore is accountable to Devore Shelter for all the cats we rescue there into our care. We do need to have copies for our own records for reference and, in the event paperwork is misplaced down the line, we always have a digital copy we can provide. Pullers generally mail their shelter paperwork via USPS to our “Paperwork Central” within 3 days of the pull date. When received, all pages of the paperwork is scanned, converted into PDFs and uploaded into our shelter paperwork file on Dropbox. Rescue partners do have the ability to access this Dropbox to check for their paperwork uploads. Turnaround time from pull to paperwork upload is usually about 2 to 3 weeks. The originals are then sent by USPS to the rescue partner at the address provided on this application. We do understand this delay isn’t always convenient and, for that reason, at the time the rescue is being coordinated on the Kitty Devore network, it is the rescue partner’s responsibility to relay their immediate need for shelter paperwork to the rescue coordinator. She, in turn, will let the puller know they need to take photographs of each page of the paperwork issued for the rescue partner’s cat(s) and remit them to the rescue partner before they mail the shelter paperwork to “Paperwork Central”. The originals will still be sent to “Paperwork Central” to be processed and will eventually be sent to rescue partners.* I understand I do not understand I understand if a cat that has been rescued by the KD team on my behalf and the rescue group I represent and the cat undergoes a snap test and the results are positive for either FeLV or FIV, the rescue group I represent still assumes responsibility and custody of the cat and will ensure the cat will be well cared for and that all their medical needs be addressed and attended to for the duration of his or her lifetime. However, if humane euthanasia is the only merciful option, we trust the rescue partner group will have the procedure performed in accordance with their rescue policies.* I understand I do not understand I understand that if a cat that has been rescued by the KD team on behalf of myself and the rescue group I represent is to be temporarily fostered by a KD team member before they can be transported to me and my rescue group or before our rescue group members can pick them up and assume custody of them, policy of vetting in accordance with KD standards may apply. In such cases and if the KD temporary foster guardian requires it to ensure the safety and health of the other cats in their household, a cat will need to have their FVRCP vaccine and possibly a snap test. Details and specifics in such cases are always discussed and negotiated between the rescue partner and an administrator of KD to come to a workable arrangement that is agreeable to both rescue groups.* I understand I do not understand I understand that there must be an agreement between the two rescue groups regarding safe and responsible transportation of the cat after rescue. KD does have reliable and responsible transporters that are either members of our team or independently contracted that we work with and we are more than happy to make arrangements for shorter distance, southern California local transportation on behalf of the rescue partner. We are also more than happy to work with a transporter of the rescue partner’s choosing. Rescue partners are responsible for funding their own transports should the transporter negotiate gas and charitable rates for mileage.* I understand I do not understand understand for longer distance transports, Kitty Devore does not allow cats to fly via cargo on commercial airlines under any circumstance and we do not advocate legs of ground transport that exceeds 3 different transporters for longer distance transports. We cannot participate in assisting transport arrangements for long distance transports unless a cat is flown in-cabin and accompanied by a guardian or transported via ground transport with 3 or less transporters. For all long distance transports, it will be up to the rescue partner to arrange and fund.* I understand I do not understand I or one of our rescue group members from the rescue group I represent will provide a humane environment, loving attention, food, litter and proper medical and physical care for the cats taken into the care of the rescue group I represent. Should a member of the KD team find proof of negligence and it’s reported to the Kitty Devore Rescue Network board of officers for evaluation and determined to be valid, the partnership between the Kitty Devore Rescue Network and the rescue partner group may be terminated* I understand I do not understand I understand the history of most cats admitted to Devore Shelter is unknown and neither I or the rescue group I represent will hold Kitty Devore Rescue Network responsible for any inadvertent exposure of the rescue partner’s other animals to unknown diseases, medical conditions or behavioral issues they may have obtained while in the shelter or prior to their admittance to the shelter system. I knowingly accept this risk on behalf of the rescue group I represent and the other animals in my rescue group’s care.* I understand I do not understand I authorize Kitty Devore Rescue Network to contact any person or entity named in this application to verify information provided. A Kitty Devore Rescue Network representative may visit my rescue group’s home base (if applicable) before this application is approved or during the time that the rescue partnership arrangement exists between my rescue group and Kitty Devore Rescue Network.* I understand I do not understand I understand that Kitty Devore Rescue Network will not be liable for any injury or damage to persons or things caused by a cat I requested be rescued on behalf of the rescue group I represent once they are in the custody of my rescue group and I will not hold Kitty Devore Rescue Network responsible for any such injury or damage.* I understand I do not understand I understand that once the cat is pulled from the shelter by a member of the KD team for me and the rescue group I represent, the cat is under the aegis of the rescue group I represent and will not be considered a KD kitty and that the cat will be subject to the vetting standards, fostering program and adoption process of my rescue group and does not need to adhere to Kitty Devore Rescue Network vetting standards, fostering program or adoption process or policies.* I understand I do not understand I understand that should the Kitty Devore Rescue Network board of officers become aware of a cat or cats pulled for a rescue partner who is in an unsafe situation and the rescue partner, after being made aware, does not act to secure the safety of the cat in question, Kitty Devore Rescue may step in and assume custody of the cat(s) and reserves the right to not return the cat to the rescue group if they feel it would be a detriment to the health and safety of the animal.* I understand I do not understand I understand that if Kitty Devore Rescue Network board of officers becomes aware of a cat that was pulled for a rescue partner but has found his or her way into a shelter once again, we will alert the rescue partner and expect they will either contact their adopter to take action to reclaim the cat or takes action, themselves, to reclaim the cat into their custody and care. Failure to do so may result in a Kitty Devore Rescue member visiting the shelter to reclaim the cat. Should this happen, the cat will not be returned to the rescue partner and the rescue partnership may be terminated.* I understand I do not understand Kitty Devore Rescue does have an expectation that all cats pulled for rescue partners continue to remain under the umbrella of protection of the rescue partner’s rescue group for the duration of the cat’s lifetime. If the rescue group has disbanded and no longer operational, we have the expectation that the individual cited as head of rescue on this application will assume responsibility for the cat. If the cited head of rescue fails to assume responsibility, Kitty Devore Rescue will do so and assume the cat into our rescue’s care.* I understand I do not understand I understand that should the Kitty Devore Rescue Network board of officers become aware of any questionable actions and activities as they pertain to their rescue operations, inhumane treatment of animals, violation of any laws or willful allowance of inhumane environmental conditions or individuals allowed to attain and maintain membership within the rescue group, we reserve the right to terminate the rescue partnership after an evaluation of all the facts of the situation and our own internal investigation.* I understand I do not understand Kitty Devore Rescue Network ApplicationToday's Date:* Month Day Year Name of Rescue Group:* Name of Head of Rescue:* First Last Name of Rescue Group Representative Applying for KD Membership:* First Last EIN:* 501c3 Status (If pending, write pending and an approximate date you expect to be approved.):* Headquarter Address of Rescue Group:* Street Address Address Line 2 City AlabamaAlaskaAmerican SamoaArizonaArkansasCaliforniaColoradoConnecticutDelawareDistrict of ColumbiaFloridaGeorgiaGuamHawaiiIdahoIllinoisIndianaIowaKansasKentuckyLouisianaMaineMarylandMassachusettsMichiganMinnesotaMississippiMissouriMontanaNebraskaNevadaNew HampshireNew JerseyNew MexicoNew YorkNorth CarolinaNorth DakotaNorthern Mariana IslandsOhioOklahomaOregonPennsylvaniaPuerto RicoRhode IslandSouth CarolinaSouth DakotaTennesseeTexasUtahU.S. Virgin IslandsVermontVirginiaWashingtonWest VirginiaWisconsinWyomingArmed Forces AmericasArmed Forces EuropeArmed Forces Pacific State ZIP Code Phone Number of Rescue Group:*Email Address of Rescue Group:* Website of Rescue Group:* Address for Head of Rescue:* Street Address Address Line 2 City AlabamaAlaskaAmerican SamoaArizonaArkansasCaliforniaColoradoConnecticutDelawareDistrict of ColumbiaFloridaGeorgiaGuamHawaiiIdahoIllinoisIndianaIowaKansasKentuckyLouisianaMaineMarylandMassachusettsMichiganMinnesotaMississippiMissouriMontanaNebraskaNevadaNew HampshireNew JerseyNew MexicoNew YorkNorth CarolinaNorth DakotaNorthern Mariana IslandsOhioOklahomaOregonPennsylvaniaPuerto RicoRhode IslandSouth CarolinaSouth DakotaTennesseeTexasUtahU.S. Virgin IslandsVermontVirginiaWashingtonWest VirginiaWisconsinWyomingArmed Forces AmericasArmed Forces EuropeArmed Forces Pacific State ZIP Code Phone Number for Head of Rescue:*Email Address for Head of Rescue:* Address for Rescue Representative Applicant:* Street Address Address Line 2 City AlabamaAlaskaAmerican SamoaArizonaArkansasCaliforniaColoradoConnecticutDelawareDistrict of ColumbiaFloridaGeorgiaGuamHawaiiIdahoIllinoisIndianaIowaKansasKentuckyLouisianaMaineMarylandMassachusettsMichiganMinnesotaMississippiMissouriMontanaNebraskaNevadaNew HampshireNew JerseyNew MexicoNew YorkNorth CarolinaNorth DakotaNorthern Mariana IslandsOhioOklahomaOregonPennsylvaniaPuerto RicoRhode IslandSouth CarolinaSouth DakotaTennesseeTexasUtahU.S. Virgin IslandsVermontVirginiaWashingtonWest VirginiaWisconsinWyomingArmed Forces AmericasArmed Forces EuropeArmed Forces Pacific State ZIP Code Phone Number for Rescue Representative Applicant:*Email Address for Rescue Representative Applicant:* When was the rescue group founded?:* Does the rescue group have a mission statement?* Yes No Mission Statement:*What kinds of animals does the rescue group focus their rescue efforts on?:* How many animals are currently in the care of the rescue group?:*Please enter a number greater than or equal to 0.Describe the rescue group’s vetting policy:*Describe the rescue group’s fostering program:*What is the rescue group’s criteria for fostering?:*What is the rescue group’s policy on circumstances where humane euthanasia would be considered for an animal in their care?:*How does the rescue group handle situations where a cat in their care tests positive for FeLV, FIV or FIP?*Describe the rescue group’s adoption process:*What is the rescue group’s criteria for an interested adopter?:*What does the rescue group consider to be a deal breaker when it comes to an interested adopter?:*What is the rescue group’s “return policy”?*Most of KD monetary exchanges are conducted via PayPal.
