Document Submission & Request for Reimbursement Please use this form to submit all documents and reimbursement requests. Documents The information you provide in the form below will go directly to Renée Mc Elwee. She will confirm she has received your request when she processes the request forms. This can take from 1 day to a full week. Please make sure your invoices are included. We will not reimburse without accompanying invoices. File format for invoices should be either JPG or PDF. Please DO NOT include spay/neuter/death certificates and letters of extension with your invoices for reimbursements requested. Please select Spay/Neuter Certificate above for instructions as to how to submit those forms. Please do not save up your invoices and then submit them all at once. Instead, it is preferred they be sent immediately following services rendered. For basic procedures we will reimburse up to: $75 for spay/neuter $50 for snap testing $15 for FVRCP vaccines $15 for Rabies vaccines Reimbursements are paid in the order they are received and sometimes, depending on our funding, it can take up to a month before you receive a reimbursement payment for your requested reimbursement. If your reimbursement request is over $100, you will be paid in installments of $100 at a time until the entire amount is completely paid in full. PayPal is our preferred method of remitting payment but, if you would prefer a check to be issued and sent to you, please indicate this on your reimbursement form. Please be assured that you will be reimbursed! We have never failed to pay reimbursement requests in full. Even a bill of $5,000 was paid back in full. Granted, it did take us 5 months to do it – but we did do it. Anything under this amount will most assuredly take less time to repay.On occasion, Kitty Devore Rescue members opt to waive their option of requesting reimbursement for expenses incurred in the process of vetting their foster cats in accordance with KD mandatory vetting requirements or after seeking medical treatment for them. For some, doing so is their way of donating to the rescue group. Although we always expect to reimburse for all expenses, we greatly appreciate the generosity of our members who are able to donate to KD in this way. If you prefer to request reimbursement for your expenses, please select Reimbursement Request above. Despite the waiver of reimbursement request, paperwork, documentation and invoices incurred for donated expenditures must still be submitted for the KD records and files. If you are choosing to donate your expenses for expenses incurred for your foster cat or for the sponsorship of another foster cat within KD Rescue, please complete this form and upload the accumulated documents of the expense and procedures here. Know that your donation is tax-deductible. Kitty Devore Rescue Network is recognized by the state of California and the IRS as a 501(c)(3) nonprofit charitable organization. When your submission is received, it will be acknowledged as such within 3 business days via email. Please keep a copy of the acknowledgement in addition to the original documentation of expense for your personal tax records.If your foster cat or kitten was pulled Rescue Only or was a Friendly on Medical Waiver, you MUST have him or her spayed or neutered, in most cases, within 30 days of the date of the pull. A failure to do so will put our Rescue Only pull rights at Devore shelter in jeopardy and can jeopardize our Regular Adoption pullers from being able to pull, too. Having Rescue Only Pull Rights suspended prevents us from helping any cats deemed Rescue Only at Devore so, understandably, this is not something we want to risk. Aside from assuring the absolute well-being and care of your foster cat, this is one of the PRIMARY responsibilities of the foster. Standard time frame for sterilization certificate submission for healthy, non-pregnant or nursing cats is 30 days from the date the cat was pulled from the shelter. For pregnant or nursing cats, the time frame is usually 60 to 90 days from the date the cat was pulled from the shelter. The date will be stated on your shelter paperwork and will be based on pregnancy or the age of the kittens at the time the family was rescued. For cats that are ill and cannot safely undergo surgery and you know they will not be able to be sterilized within the 30 day period, you MUST get a letter from a veterinarian’s office on their letterhead stating your cat cannot be spayed or neutered and why with a projected date for the procedure. For cats that have been taken in to see a veterinarian and found to be too old to spay or neuter or the vet has determined they have already been spayed or neutered, you MUST obtain a written statement from the veterinarian on the veterinarian’s letterhead confirming this. If a cat dies in your care before a certificate of sterilization can be obtained, you MUST hold onto the cat and, depending on the veterinarian and what they require, take the remains into their office to obtain a death certificate or statement on their letterhead that states the cat is, in fact, deceased. Please make certain the cat’s description as seen on the shelter paperwork and the animal ID number appears on the certificate or veterinary letter. Please make sure you write the intake number on the certificate and the original name of your foster cat in addition to the secondary name you may have given them after they were rescued and once they were in your care. KD files go by the intake numbers and the original names given. An invoice that shows a cat has been spayed and you paid for it DOES NOT suffice as a valid certificate. The document must have the cat’s name, shelter ID number, coloring and fur length according to their Devore listing, age (which may be different from what Devore speculated it was) and gender (again, this may be different from what Devore speculated it was). If possible, please try to avoid having your name appear on any certificate or letter’s of extension or previous spay/neuter confirmation letter if someone else other than you pulled the cat from the shelter. The only way we can be reimbursed for spay/neuter deposits is if the puller’s name appears on the paperwork or if the information areas are left blank. However, if your vet insists on adding your name, address and contact information to the paperwork, don’t stress. All certificates, when submitted to Renée Mc Elwee are run through Photoshop to be stamped with KD’s “Submission Stamp”. Anything can be deleted out or corrected as necessary.
